A clear photo of all site members wearing full PPE (helmet, reflective vest, and safety shoes) must be provided. The photo should include at least one member for every two team members present (e.g., if the total team is four, the photo must show three members; if the team is two, the photo must show one member). HSE compliance will be evaluated based on the Scope of Work (SOW) and alignment with the number of team members on site.

The Site Supervisor is responsible to conduct daily safety talks with teams on a topic relevant to the tasks they will be performing for the company. Think about the activities, tools or equipment that you will be doing or using today and select at least one activity, tool or equipment to discuss as a topic in your Toolbox Talk.

By signing I acknowledge:
  1. The risk assessment was discussed with me and I am aware of all dangers on site and the controls needed for the hazards and environmental controls in place.
  2. I agree to obey all safety, health, environmental and quality rules, instructions and procedures.
  3. The required PPE has been issued, inspected and is in good working condition and I am aware of the limitations there of (e.g. Hard hats will not protect wearer for heavy objects from height.)
  4. I will not perform any tasks outside my scope of work, or beyond that for which I am trained for.
  5. I will report all incidents, accidents and near misses to the Site Supervisor before leaving site.
  6. I also acknowledge that I will comply with the Absolute Rules listed below.
