PROOF OF COMMUNICATION By signing this document I acknowledge that:

1. The risk assessment was discussed with me and I am aware of all dangers on site and the controls needed for the hazards and environmental controls in place.
2. I agree to obey all safety, health, environmental and quality rules, instructions and procedures.
3. The required PPE has been issued, inspected and is in good working condition and I am aware of the limitations there of (e.g. Hard hats will not protect wearers for heavy objects from height.)
4. I will not perform any tasks outside my scope of work, or beyond that for which I am trained for.
5. I will report all incidents, accidents and near misses to the Site Supervisor before leaving the site.
6. I also acknowledge that I will comply with the Absolute Rules listed below.
